The CHF assessment is useful for people at risk for Congestive Heart Failure. It is a series of five simple multiple choice questions. The answers are entered into a Care Note, emailed at the end of the day to the users set up as primary caregivers and account managers, and are available on the GCManage dashboard for your Loved One's account. CHF Assessment
What is the CHF Assessment?
The CHF assessment is useful for people at risk for Congestive Heart Failure. It is a series of five simple multiple choice questions. The answers are entered into a Care Note, emailed at the end of the day to the users set up as primary caregivers and account managers, and are available on the GCManage dashboard for your Loved One's account. Answer each question by pressing your preferred answer. The assessment ends automatically after all the questions are answered.
