Assessment

The CHF assessment is useful for people at risk for Congestive Heart Failure. It is a series of five simple multiple choice questions. The answers are entered into a Care Note, emailed at the end of the day to the users set up as primary caregivers and account managers, and available on the GCManage dashboard for your Loved One’s account.
